    Member Care Coordinator | Hybrid | Springfield, IL | Contract to Hire Walker Healthforce is seeking a Member Care Coordinator for a healthcare client located in Springfield, IL.

    This is a contract position.

    Walker Healthforce Standard Perks:

    Weekly pay via Direct Deposit Medical, Dental, and Vision Available 401K JOB DESCRIPTION Conduct home health assessments and system updates Perform outreach and follow-up with members on their healthcare plans and conduct check-ins with members to review care plan goals Build relationships with members to ensure compliance with care plans and alert Case Managers to issues promptly Inform and educate members on their program, using provided scripts if necessary Encourage member usage of programs and arrange appointments and additional services Generate and send appropriate correspondence to members manually, electronically, or by phone Maintain production requirements based on department needs Ensuring member or provider questions are addressed or directed internally Support and maintain communication with internal areas such as Marketing and Provider Affairs Adhere to HIPAA, Diversity Principles, Corporate Integrity, and Compliance Program policies Maintain complete confidentiality of company business Keep management informed of developments and perform special projects as required Written correspondence to members and to other department personnel and coaching skills, including motivational interviewing, to educate members on medical issues JOB REQUIREMENTS 3+ years care coordination for a state managed or waiver program OR 3 years managed care systems experience.
