Description

A Care Management Coordinator will be responsible for assisting outpatient counseling providers and psychiatric nurse practitioners with care management support, maintaining compliance with their caseloads, coordination of care with primary care physicians and other health care agencies, and completing outreach to clients that are not engaged in services.


Knowledge and Skills:


  • Communication skills: Due to the nature of behavioral health, communication must be professional, clear, calm, and sensitive at all times regardless of the circumstances
  • Multitasking skills:
Must develop and demonstrate excellent multi-tasking skills to effectively complete multiple required tasks

  • Phone and computer literacy:
Must maintain phone and computer handling skills. Typing by touch skills are necessary in order to properly document.


Duties:


  • Monitoring caseloads of multiple outpatient providers for various documentation requirements and regular client attendance
  • Communicating with providers, supervisors, and clients to relay information regarding documentation requirements, attendance, treatment, etc.
  • Outreaching clients via telephone to for varied purposes, including following up on attendance, scheduling for ongoing treatment, and requesting updated primary care physician information
  • Assisting outpatient providers with completing the postcrisis/hospitalization requirements for clients who have been in crisis or hospitalized.
  • Monitoring client transfers as needed to ensure that clients successfully connect with new ongoing provider(s), updating the client chart and communicating to all involved parties when a transfer has or has not successfully occurred.
  • Meeting individually with new counseling clients for warm transfer to ongoing provider, documentation in EMR, and communicating all details of warm transfer to intake provider, ongoing provider, and all other members of the client's clinical team.
  • Working with other departments (scheduling, front desk, PNP, billing, insurance verification) to troubleshoot issues and ensure that all details are addressed completely and in a timely manner.
  • Maintaining coordination of care requirements which must be upheld for various insurance payors
  • Running reports in the EMR system and from other data systems to facilitate coordination of care via efax, monitor for successful transmission of information, and document within patient charts
  • Reviewing charts for various elements pertinent to the completion of coordination of care (releases of information, current primary care physician contact information, treatment information, etc.)
  • Contacting insurance plan to obtain primary care physician information for clients who are unsure of their assigned PCP
  • Being crosstrained to cover the front desk as needed
  • Become proficient in EMR and any other software introduced by CPR for clinical workAttend and participate in clinical staffings/meetings with counselors and nurse practitioners as needed
  • Consistently observe HIPAAcompliant patient confidentiality measures.
  • Consistently demonstrate CPR's core values.
  • Other duties as assigned

Qualifications and Education Requirements- High School or GED Diploma required- 2 year degree or some college preferred- 2 years behavioral health experience preferred


Other Items:


  • Must comply with training and supervision requirements during the first 90 days of employment, and throughout employment.
  • Medicare/Medicaid sanctions, criminal violations, subsequent malpractice actions and any settlement made on behalf of the practitioner.
  • Valid driver's license and auto insurance must be maintained at all times. Must maintain current CPR/First Aid card and biannual TB test.
  • All expirables to be renewed and submitted 30 days prior to expiration date.
