About the position:
The purpose of this position is to oversee the care of our clients as well as build and maintain strong relationships with our various partners throughout the community.
Responsibilities:
- Supervisory field visits, ensuring quality control and appropriateness of care
- Provide client assessments, reassessments, and referrals to company vetted partners as needed
- Participate in the oncall rotation after normal business hours and weekends
- Maintain relationships with discharge planners, case managers, social workers, and other health care professionals who utilize home care services
- Attend an occasional networking event as a representative of the company, as needed
- Willingness to provide hands on care for our clients when needed
- Perform general office/administrative staff member duties as needed
About You:
- Associate's Degree, or higher required
- Bachelor's Degree, or higher preferred
- Experience with care or case management
- Technologically savvy, and computer literate
- Elder services experience is a plus
Pay:
$37, $45,000.00 per year

Ability to Relocate:
