Requirements:

Duties:
- Coordinate patient services and ensure the delivery of highquality medical care
- Manage medical records and documentation efficiently
- Provide direct patient care, including medicalsurgical and infectious disease care
- Demonstrate a deep understanding of physiology, medical terminology, and critical care practices
- Collaborate with healthcare professionals to optimize patient outcomes
- Conduct comprehensive initial assessments and develop personalized care plans for assigned members.
- Establish and maintain strong rapport with members, managing a caseload of up to 25 individuals.
- Assist members in accessing communitybased resources, scheduling medical appointments, and completing necessary referrals for both community and health services.
- Document all interactions and activities accurately using the designated health portal.
- Conduct outreach tasks and conduct facetoface visits with members in a clinical setting.
