Full Description

VitalCheck Wellness, on behalf of Photon Medical Group, is seeking a culturally sensitive and compassionate
Licensed Master Social Worker (LMSW) to join our team.

Patient care will be provided in-person and virtually at one of NYC Health + Hospitals' Humanitarian Emergency Response and Relief Centers (HERRCs) making a difference in the lives of migrants.

Responsibilities

Conduct culturally competent shortterm counseling, crisis intervention, risk assessment, safety planning, and psychoeducation, including:
Crisis intervention
Identify need and make referrals, for outpatient/inpatient care and hospital transport
Trauma triage/trauma informed assessment
Peer support
Conducting emergency evaluation/assessment for:
Mental health and mental health crises
Substance use/disorder
Occupational needs
Domestic violence
Advocacy on behalf of the family when working with NYC Administration for Children's Services or other preventative services.
Communicating and collaborating with external service providers to enhance engagement and ensure quality of service delivery.
Providing education and training to other staff on mental health and holistic support needs and benefits.
Facilitating family meetings with the family and HERRC staff to discuss planning efforts and progress.
Coordinating with emergency services, hospital social workers, and discharges, particularly around laboring individuals and newborns.
Coordinating on domestic violence cases, particularly in instances where a 911 call must be made.
Handling transfers to domestic violence shelter.

Requirements:

Master's in Social Services, Mental Health Counseling, and/or Marriage and Family Therapy.
Licensed Clinical Social Worker (LCSW), Master of Social Work (MSW), or Licensed Master Social Worker (LMSW) certification.
Experience in a healthcare or hospital setting is highly preferred.
Bilingual in Spanish a Plus
Knowledge of New York State regulations and guidelines for social work practice.
Strong interpersonal and communication skills.
Ability to work effectively in a fastpaced and dynamic environment.
Empathy and sensitivity to the needs of diverse patient populations.
Proficiency in electronic medical record systems and documentation.
Must be able to commute to New York City and Boroughs.

The Program is a collaboration between VitalCheck Wellness and hospitals, independent medical practices and individual physicians designed to bring high-quality preventive healthcare to patients on site at their place of work or via telemedicine or at other locations where the patients are located.

As part of the Program, VitalCheck Wellness also establishes and manages fixed onsite clinics for clients at their offices or at other locations where the patients are located.

VitalCheck Wellness provides administrative services and proprietary technology to clients and to healthcare providers.

Pay:
$45.00 per hour
