What you'll be doing as a Memory Care Coach:
- The Memory Care Coach engages with the patient, their care partner(s), medical provider, primary care provider, Advanced Practice Nurse and Interdisciplinary Team (IDT) to provide a Cognitive Assessment as part of the Comprehensive Care Planning process.
- Conducts memory care coaching sessions, averaging at least one hour per week, per patient/care partner(s).
- Provides psychoeducational skillbuilding and education to patients, care partner(s) and family members relative to the impact and implications of Dementia and patient care.
- Coordinates and facilitates weekly support groups for community members to educate on the impacts and implications of Dementia.
- Fosters connection of patients and care partner(s) to a Memory Care licensed clinical therapist.
- Serves as a support person for the patient/care partner(s) on an ongoing basis.
- Makes referrals to community resources to meet the needs of the patients/care partner(s), when indicated
- Attends IDT meetings and contributes positively to patient care planning.
- Assists patient/care partner(s) and other team members in providing continuity of care.
- Performs speaking engagements and provides inservice education on Alzheimer's disease, other types of dementia and memory care management to contractors, referral sources and other appropriate groups, as required.
- Performs other duties, as assigned.
What we're looking for:
- Experience working in a health care setting with dementia patients, preferred.
- Experience with inhome caregiving, preferred.
- Possesses understanding of the philosophy and principles of dementia care, with counseling, coaching, or teaching experience, preferred.
- Must have effective communication, comprehension, documentation, interpersonal and computer skills.
- Able to work within an interdisciplinary setting.
- A professional attitude and demeanor and be able to communicate effectively and relate courteously and cooperatively with patients, caregivers, colleagues, supervisor, coworkers and all others is required.
Education and Experience:
- Possess an Associate's degree in a healthrelated field; Bachelor's degree in social work or healthrelated field, preferred.
- Able to obtain Certified Dementia Practitioner (CDP) certification from the National Council of Certified Dementia Practitioners within the first six months of employment.
Physical Requirements:
Position requires sitting, standing, walking, bending, pushing/pulling, kneeling, squatting, reaching, twisting.
Also required is driving, writing, speaking, relating to others, making independent decisions, listening and finger dexterity, and the ability to lift and carry.
