    We are looking for a Home Health Licensed Practical Nurse to be part of our dedicated team of nurses in Mount Vernon, IL. This position will focus on serving patients in the Vandalia area.

    The Licensed Practical Nurse (LPN) in Home Health is tasked with delivering high-quality patient care and administering specialized medical services in the patient's home.

    • Deliver services within the boundaries set by state nursing regulations, following the care plan and collaborating with other healthcare professionals. It is important not to modify the care plan.
    • Educate the patient's family on caregiving and maintaining a safe environment for the patient.
    • Observe, document, and communicate the patient's response to treatment and any changes in their condition to the supervising nurse or physician on a daily basis.

    License Requirements

    • Up-to-date LPN license in the practicing state.
    • Valid Driver's License, car insurance, and access to a reliable vehicle or public transportation.
    • CPR Certification is mandatory.
