Under the direction of the Director of Nursing and MDS Lead, the _MDS Coordinator (Registered Nurse RN or Licensed Practical Nurse LPN)_** is responsible for implementing the federally mandated assessment tool to identify needs / concerns of the resident and to coordinate a plan of rare to assist the resident toward optimal wellness based on the highest practicable level of functioning.

We are a 179-bed Skilled Nursing facility in Lutz, Florida and is designed for culture change based on the national movement for more individualized care.

We focus on the individual needs of each of our patients and partner with physicians, patients, and their families to ensure successful patient outcomes.

MDS COORDINATOR REGISTERED NURSE / LICENSED PRACTICAL NURSE QUALIFICATIONS:

Must possess current and valid
_Registered Nurse (RN) _License or be a
_Licensed Practical Nurse (LPN)_ in the state of Florida
Must be able to relate positively and favorably to residents, families and outside agencies and to work cooperatively with other associates
Prior MDS / PDPM Experience Required
