    Pharmacist, Outpatient Oncology and Medical Infusion - Waltham Description

    GENERAL SUMMARY/ OVERVIEW STATEMENT:

    We have an exciting opportunity to make a significant impact on patient care in the community oncology and medicine setting Join the Mass General Waltham team as a Pharmacist. A key area of responsibility will be to ensure provision of safe, appropriate, timely, and cost-effective pharmacotherapy to our patients in accordance with evidence-based literature, regulations governing pharmacy practice, and institutional policies and procedures. The pharmacist will also function as a drug information resource to the other health care providers and help resolve complex therapeutic issues as an integrated multidisciplinary team member..     PRINCIPAL DUTIES AND RESPONSIBILITIES:

    Job-specific Responsibilities

    Oncology and Medical Infusion

    - Monitors patient treatment and medication use daily for appropriateness based on current clinical guidelines.

    - Assesses all medication orders to ensure they meet established standards and practice guidelines of Mass General Hospital and Cancer Center. Reports order discrepancies to appropriate staff promptly.

    -Utilizes computerized systems to process medication orders, document interventions and safety reports to maintain and support safe and consistent medication administration

    -Correlates patient laboratory results to prescribed therapy assessing need for dosing changes based on pharmacodynamic or pharmacokinetic alterations in drug disposition. Reports concerns and suggestions to clinical staff in a timely and effective manner to ensure patient safety and positive clinical outcomes and documents interventions in the medical record as appropriate.

    -Evaluates/reconciles all current patient medications including over-the-counter and herbal medication for drug interactions.

    -Educates patients and their families to ensure their understanding of the goal, administration schedule and toxicity management of chemotherapy treatment and documents the activity and interventions in the medical record.

    -Accesses current clinical trial documents when reviewing clinical trial orders to ensure accuracy.

    -Maintains investigational medication logs.

    -completes required training and competencies as defined by leadership

    Operating Room

    - Coordinates and assures the timely and accurate provision of pharmaceutical services.

    --coordinates preparation/procurement/distribution of specific patient medication orders and/or solutions for OR use.

    - Responsible for control, distribution and monitoring of the use of controlled substances in the operating room area.

    General:

    -Advises clinicians on drug selection based on current clinical guidelines

    -Participates in nursing education activities

    -

    -participates in performance improvement activities to evaluate the safety, effectiveness and outcome of services delivered.

    -orders and stocks medication and supplies

    -supervises the preparation and distribution of medication

    supervises the activities of the pharmacy technician

    -

    provides support and expertise for use and maintenance of automated medication dispensing systems

    -supports a distribution system that complies with state and federal regulations

    -maintains the system of controlled substance inventory

    -identifies, recommends treatment of and reports adverse drug reactions

    -identifies and reports therapy errors

    -supervises the activities of the technician

    -trains other pharmacists, technicians and students

    -assists in the development of policy and procedures

    -Seeks out and applies new pharmaceutical concepts and information as available. Disseminates current information to support other professional staff.

    Manufactures sterile products according to current guidelines (ASHP, USP, NIOSH)

    -Adheres to safety requirements in the handling and preparation of chemotherapy and other hazardous agents.

    Qualifications

    SKILLS/ABILITIES/COMPETENCIES REQUIRED:

    -computer literacy

    LICENSES, CERTIFICATIONS, and/or REGISTRATIONS:

    Current pharmacy licensure by the Commonwealth of Massachusetts

    Maintenance of continuing education as required by law

    EDUCATION:

    Graduation from an accredited college of pharmacy (BS Pharmacy or Doctor of Pharmacy)

    EXPERIENCE:

    Minimum of 1 year in hospital pharmacy, oncology and/or operating room experience required

    SUPERVISORY RESPONSIBILITY:

    N/A

    FISCAL RESPONSIBILITY (if applicable):

    -enforces the formulary system

    -minimizes drug waste

    -dispenses medications according to accepted standards of practice and evidence

    WORKING CONDITIONS:

    Daily exposure to the mental/emotional stress related to the oncology experience(s) of patients/families.Risk of exposure to hazardous materials, known carcinogens and risk of physical injury.

    Able to lift up to 20 lbs.

    Able to stand or sit for extended periods of time.
