Work Shifts:


8:00 A.M - 4:30 P.M

Purpose:


Under the direct supervision of a Licensed Nurse, Physician or other healthcare professional performs a variety of clinical and clerical duties associated with the care of patients in the Out-Patient Department.


Specific Duties and Responsibilities:

The Patient Care Associate (PCA) is charged with and responsible for:

  • Ensures compliance with National Patient Safety Goals (NPSG): 2 patient identifications, HIPPA, infection control, communication, etc.
  • Takes vital signs and performs any other screenings, PHQ9, GAD, SDOH, smoking screen, etc. that are essential and recommended for the visit.
  • Measures weight and height, reviewing previous result prior to documenting and reports findings to licensed professional.
  • Performs head circumference vision and hearing screening (Pediatrics), and urine dipsticks (WHC) and provides licensed professional with documented results.
  • Completes previsit planning (PVP) 23 days in advance, reviewing Care Gap elements and communicates directly with assigned provider.
  • Alerts Front Desk staff if patient is not keeping their appointment, in order to improve patient access.
  • Ensures that equipment needed for patient care, glucometer, retinal camera, A1C, blood glucose monitor, B/P machine, urinalysis machine, urine pregnancy test machine, etc. are calibrated per protocol and with current prevention and maintenance sticker.
  • Maintains calibration logs, consistent with Laboratory & Pathology standard operation procedures (SOP).
  • Performs point of care testing (POC), electrocardiographs, retinal screening reporting abnormal results to licensed professional.
  • Prepares, labels, and collects various specimens for testing, including phlebotomy.
  • Instructs patients in obtaining specimens for laboratory testing. i.e FIT and urine.
  • Performs the necessary clerical duties involved in the process of a patient care visit, including but not limited to appointments, referrals, chart maintenance, and data entry.
  • Actively participates in daily huddles.
  • Assists in outreach of Patent Centered Medical Home (PCMH) patients with other team members.
  • Performs the necessary documentation utilizing manual or electronic procedures.
  • Administers Basic Life Support (BLS) during emergencies.
  • Maintains inventory of necessary stock and supplies including nourishment and prepares examination rooms appropriately.
  • Escorts and transfers patients to various departments as necessary.
  • Exhibits courteous and effective communication with all members of the interdisciplinary team members, patients and families with respect to cultural diversity.
  • Performs the basic functions of a nursing assistant in the Office Practice areas, including chaperoning providers.
  • Reinforces instructions given to patient/family by licensed professionals.
  • Provides care to patients throughout the growth and development continuum.

Minimum Qualifications:


  • High school diploma or its educational equivalent, approved by a State's Department of Education or a recognized accrediting organization; and
  • Successful completion of a NYS Department of Health prescribed training program for Phlebotomy, satisfactory to the Laboratory Director; and
  • Completion of a health care setting-based training program in the performance of electrocardiographs and basic life support procedures including certification in Cardio-Pulmonary Resuscitation.

Special Note:


In addition to meeting the qualifications above, assignment to a long-term care facility requires a New York State Nurse's Aide Certificate.
