- Lead population health initiatives
- Conduct in-home assessments and follow-ups
- Provide patient education on care plans and lifestyle changes
- Collaborate with cardiologists and nurse practitioners
- Deliver home-based care to patients with heart failure
- Act as a clinical resource to practice staff
- 1-3 years of care coordination experience
- Graduate of an accredited school of nursing, BSN preferred
- Current NJ license and CPR certification
