- Working through numerous different spreadsheets and creating referrals
- Outreaching to members to assist them with identified care coordination needs
- Documenting all interaction with members/providers/other team members

Job Description:

Required Skills/Experience:
Experience in behavioral health settings
Experience & knowledge in community resources available
Electronic medical record use/documentation
Preferred Skills/ Experience:
Care/treatment plan development
Engaging challenging populations
Experience with assessments like PHQ9, GAD7, Vanderbilt
Education Requirement:
Bachelor's Degree in relevant field (Social Work, Psychology, or similar field)
Describe the performance expectations/metrics for this individual and their team:
LOS:
90 days
o Maintain up to date Tru
Care Tasks:
No tasks overdue greater than 7 days
o Enrollments per Month:
Average of 40 new enrollments per month
o Enrollment Completion Efficiency:
Timely enrollment completion with average completion within one week (7 business days) of start date
o Call Volume to Meet Outreach/Engagement/
Enrollment Expectations:
Average of 300 calls per month
Audit Standards include:
o Maintaining an average of 90% on audits completed for the calendar year
o Quality documentation- documentation is thorough, easy to read/follow, etc.
o Quality care plans- elements are related to each other, member focused, etc.
o Appropriate referrals to resources to support member needs
o Appropriate case acuity designation
o Appropriate case duration
o Processes are appropriately followed
