Full-time Nurse Practitioner in Family Medicine at Ascension Medical Group Heritage Family Physicians
Ascension Medical Group is looking for a dynamic Nurse Practitioner to join their team. This is a fantastic chance for the right candidate to work in a well-established practice and offer primary care services to patients of all ages.
Position Highlights:
- Well-established practice
- Will manage own patient panel
- Join a team of three physicians and one other advanced practice provider
- Onsite primary care and lab services
- Monday to Thursday schedule from 7 am to 5 pm, Fridays from 8 am to noon
Responsibilities:
- Gather patient history and perform assessments through observation, interviews, and examinations
- Order, carry out, and interpret diagnostic studies
- Conduct preventative health assessments, screenings, immunizations, and patient care
- Provide direct treatment and management of health conditions through referrals to other healthcare providers or community resources
- Counsel and educate patients and families on preventative health, treatment options, and community resources
- Document healthcare delivery and nursing processes according to specialty area standards and network nursing policy
