Commonwealth Mental Health and Wellness Center (CMHWC) is currently seeking experienced applicants for a Mental Health Counselor Position to provide care in Therapeutic Training & Support (TT&S).

CMHWC offers care to clients of all ages, and service culturally diverse communities. Individuals fluent in additional languages are strongly encouraged to apply.


Responsibilities:

• Provide Mental Health Service of Therapeutic Training & Support (TT&S) working as part of an In-Home Therapy (IHT) team in collaboration with a Clinician, for the purpose of treating a youth's mental health, behavioral and emotional needs,
• Aid in developing positive social and behavioral skills
• Work oneonone with the youth, utilizing information from client's Guardian & In Home Therapy (IHT) Provider, in order to implement treatment goals
• Maintain client's mental health records, history, progress notes, assessments, disclosures, treatment plan etc.
• Ability to work both independently and as a Team
• Maintain excellent communication between all parties of a client's Care Team
• Keep all client information confidential, according to the Health Insurance Portability and Accountability Act (HIPAA).
• Identify community resources needed to support and sustain achievement of client's treatment plan goals
