- Illnesses or conditions to consider Chronic heart failure, reduced mobility, obesity, diabetes
- What care or services are needed? Injections, assistance with personal care or hygiene, blood extractions
- Frequency of service needed Daily
- Indicate the desired start date or approximate dates
- What time do you need the service? Late afternoon (18:00 - 21:00)
- What days do you need the service? Every day of the week
