    ESSENTIAL FUNCTIONS: Responsible for providing housing information, referral, advocacy, and support to eligible clients in support of the goals and objectives outlined within the departments housing programs. Assists families with housing needs to access all available assistance programs to determine eligibility and receive housing assistance. Also, helps families address barriers to maintaining safe housing and preventing homelessness whenever possible. Serves families in Central Falls as assigned by, and under the direct supervision of, the Program Supervisor.

    SPECIFIC DUTIES/RESPONSIBILITIES:

    Assessment and Planning--ascertaining a family's specific needs and creating a "Housing Plan" which will include needed interventions to ensure the safety of all household members as well as measurable goals and specific time lines for completion.

    Service Coordination/Housing-Related Education--Works with each family to gather and coordinate supports and services needed. Provides education regarding:

    teaches families the skills for successful tenancy and of being a good tenant;

    household management including budgeting;

    creating a healthy living environment such as the recognition and remediation of health and safety hazards (such as for asthma-related conditions); and

    window safety for small children.

    Advocacy and Intervention--Understands and helps families develop plans to address the underlying issues that may disrupt housing, such as unemployment, mental health or substance abuse issues, health concerns, immigration status, etc. It is also important to understand the housing issues themselves that disrupt permanence and self-sufficiency -- such as landlord or neighbor disputes or lack of understanding of the tenancy process.

    Coordinates service provision with other housing and support service providers and systems across the state.

    Stays up to date on housing resources in Central Falls and throughout the state of Rhode Island to improve access for clients.

    Develops ongoing relationships with culturally specific programs and resources to ensure access for underserved individuals

    Assists with outreach and engagement activities with a wide range of housing and community organizations to promote awareness of and access to services.

    Collects data as required by funding sources.

    Completes all appropriate paperwork and documentation as required.

    Attends regular weekly supervision meetings.

    Maintains appropriate standards of confidentiality in accordance with HIPPA and agency guidelines.

    QUALIFICATIONS:

    Minimum of one year experience working with adults and/or families required.

    Bilingual English/Spanish required.

    Experience working within the housing system in Rhode Island preferred.

    Must have excellent communication and interpersonal skills.

    Ability to work independently and as a member of a Team required.

    Possession of valid drivers license, reliable transportation and proof of current automobile insurance is required.

    PHYSICAL REQUIREMENTS: This position may require community visits, employees in this position must have the ability to:

    Travel to and from community locations and office sites, which could include using walkways, stairs and/or elevators.

    Ability to lift up to 20lbs.

    Ability to communicate effectively.

    Receive a Sign-on Bonus in the amount of $500.00 within 6 months of employment
