Step into a vital role within our hospital where you'll oversee the Social Services Department, ensuring exceptional care coordination and patient support services.

This position reports directly to the Director of Case Management.


Role and Responsibilities:


  • Oversee the operations of the Social Services Department, ensuring compliance with federal, state, and local regulations.
  • Develop and implement policies, procedures, and quality improvement programs.
  • Manage department budget and staffing, ensuring efficient and effective service delivery.
  • Responsible for staff engagement, quality, and performance improvement activities, and maintaining positive employee relations.
  • Work closely with physicians and other healthcare professionals to ensure collaborative and comprehensive patient care.

Qualifications:


  • Master's Degree in Social Work (MSW) required.
  • LCSW (Licensed Clinical Social Worker) preferred; mandatory for CA applicants.
  • At least 2 years of experience in an acute care setting with a strong background in trauma, stroke care, and homelessness case management.
  • Proven experience in managing social issues related to healthcare, including discharge planning and regulatory compliance.

Position:
LCSW CA License Required (must have license prior to submission).


Experience Required:
Acute care experience is a must.


Key Responsibilities:


  • Understanding law regarding discharging homeless patients.
  • Conducting trauma assessments.
  • Managing social issues.
  • Handling trauma and stroke processes and assessments.
  • Managing homeless and addicted patients.
  • Experience with discharge planning.
  • Knowledge of BH5150 holds and CA/Santa Clara County regulations.
  • Familiarity with CA homeless discharge from acute care policy.
  • Understanding agencies' processes (APS/CPS).
  • Mandate reports handling.
  • Experience in trauma and stroke patient care.
  • Acute care hospital experience is essential.
- Knowledge of child protection and adult services laws.
