JOB DESCRIPTION.
The Housing Navigator provides housing related community support to clients with identified housing needs through
coordination with Neighborhood Navigators and ECM services. Main duties include conducting personal assessments,
guiding individuals in developing housing care plans and goals, and connecting individuals to resources and support in
the community, all with the goal of providing housing tenancy and sustaining services.
JOB COMPLEXITY.
Provide connections to community resources to promote health, address healthcare barriers, receive translation/interpretation services, or address health-related social needs
- Contact eligible members to assist in development, implementation, evaluation, and revision of Assessments and individualized Care Plans and objectives, which may include accessing social services such as communitybased resources for housing, food, employment, etc.
- Attend team trainings including HIPAA, secure online case management system, data sharing and management, reporting, interpretation services and other administrative support
MINIMUM COMPETENCIES.
- Must be 18 years of age
- Minimum of a High School Diploma or GED required
- Experience providing case management/community health work services to underserved or special needs populations with varied and complex health, behavioral health, economic and educational circumstances.
- Personal experience living or working in a community to be served, including awareness of availableresources, community conditions, barriers, and assets.
- Must have reliable transportation to travel through San Diego County; when using personal vehicle must maintain valid driver's license and current auto insurance
- Bilingual (Spanish/English) Required
COMPENSATION.

Starting Rate:$22.00 annually (_all new hires begin at starting salary regardless of experience_)
