    Position Overview:
    The Outreach Specialist will coordinate client outreach efforts for Health Homes programs throughout Staten Island.

    Key responsibilities include conducting home visits to identify and enroll eligible members, re-engaging lost clients, collaborating with community providers, documenting encounters, and supporting operations.


    Compensation:
    $22.00/HR


    Location:
    Staten Island, New York


    Schedule:
    Monday - Friday, 9:00 am - 5:00 pm


    Responsibilities:


    • Work with supervisor to identify outreach clients
    • Conduct home visits, phone calls, letters to enroll eligible members
    • Complete progress notes, screenings and assessments
    • Drive agency vehicle to perform community outreach
    • Document all activities in program database

    Qualifications:

    • High school diploma or equivalent
    • Experience with homeless/HIV groups preferred
    • Bilingual Spanish/English a plus
    • Valid NY driver's license with clean record
    • Strong communication and documentation skills
