_ Community Health Action of Staten Island (CHASI)__ drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities - the poor and working poor, low-income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, and the LGBTQ community._

DUTIES & RESPONSIBILITIES:


  • Establish a working relationship with the local Department of Social Services (DSS) and/or Human Resources Administration (HRA)
  • Responsible for all SNAP outreach activities to increase access and participation in the SNAP Program, including, but not limited to
:


  • Create and produce outreach materials in order to educate potentially eligible individuals about SNAP (food stamps)
  • Coordinate education presentations for potentially SNAPeligible community members, stakeholders, and leaders through community events, presentations, and tabling opportunities in the community
  • Outreach to and prescreen clients at CHASI's Food Pantry and Mobile Food Pantry locations for eligibility in the program
  • Prescreen individuals and households for SNAP eligibility, providing an estimated SNAP benefit amount, if able
  • Collaborate with local communitybased organizations, educating them about the importance of NOEP and SNAP
  • Confidentially track client information and outcomes for reporting purposes
  • Attend all mandatory trainings and webinars
  • Report outcomes via Hunger Solutions New York's online Contract Management System and complete other data entry and reporting as required by funder or agency on a timely basis
  • Conduct regular, comprehensive client documentation reviews for quality and completeness
  • Achieve annual program targets as assigned by Hunger Solutions New York
  • Perform all functions in alignment with CHASI's Mission, Vision, and Core Values

Requirements:


QUALIFICATIONS:


  • At least 2 years of experience in the human services field required; experience providing communitybased outreach, making linkages and referrals, and/or establishing community partnerships highly preferred
  • High School Diploma or Equivalency required.
  • Familiarity with resources on Staten Island highly preferred
  • Bilingual (English/Spanish) preferred
  • Must have a reliable means of transportation to get to and from community events and presentations in a timely manner.
  • Interviewing and assessment skills
  • Excellent oral and written communications skills; comfort with public speaking; ability to manage communications tactfully and professionally with a high attention to detail and accuracy
  • Strong organizational skills and attention to detail with an ability to handle multiple projects and activities while meeting established timelines
  • Ability to work independently while also bring a team player with an ability to work on diverse teams with shared responsibility and accountability; ability to contribute to a positive, productive, open, and supportive environment
  • Must have reliable means of transportation
