Description

Inspire Nursing Services-a rapidly growing home health agency is looking for experienced RN's to complete home health start of care, follow up and home health aide supervisory visits for patients located within the Lake & Porter counties.

Full-time, part-time and PRN positions available. _
Will also consider applicants interested in contracting as a 1099 employee._

SOC rate-$90 ROC-$75 Skilled visit-$65 (Weekly Pay)

Qualifications

Current Indiana License and graduate of an approved school of nursing

- Must have prior home health care experience

Recent two step PPD or equivalent blood test
Experience with AXXESSCARE charting software preferred

-Demonstrated competency with CMS charting requirements

Ability to exercise initiative and independent judgement
Current BLS Card

Responsibilities

Performs the initial evaluation visit and regularly reevaluates the patient's nursing needs.
Initiates the plan of care and necessary revisions.
Performs services in accordance with the plan of care.
Prepares clinical and progress notes for each patient visit and summaries of in a timely manner as per Agency policy.
Counsels the patient and family/significant others in meeting nursing and related needs.
Processes orders and notifies physician of patient needs and changes in condition.
Completes certification/recertification orders and discharge summaries.
Supervises and teaches other nursing personnel.
Conducts patient care conferences on patient assigned to his/her care.
Communicate effectively with interdisciplinary team members
