    Rounding Geriatric Primary Care
    Twin Cities Metro AreaSalary: $200, $300,000.00 per year

    Our main objective is to become our patients' primary caregiver throughout their lifetime, fostering enduring connections with them to assist them in leading the healthiest lives possible. At Twin Cities Physicians, you will add a personal touch in steering your patients' care plan, extending our services to various senior living communities.

    Rounding Geriatric Primary Care Physician Overview:
    • Deliver on-site primary care services, conducting 12-18 daily visits, to patients in senior living environments
    • Establish and cultivate trusting relationships with your patients throughout their care journey
    • Work closely with APPs to develop care strategies, provide recommendations, and educate for more efficient care
    • Sustain patient care on-site; minimizing medication complexities and preventing patient hospitalizations

    Rounding Geriatric Primary Care Physician Profile:
    • Must have an Unrestricted Physician license issued by the MN Board of Medical Practice
    • Demonstrate a passion for, and understanding of, the best care practices for the Geriatric population
    • Previous experience in rounding in Senior Living settings, or as a Hospitalist, would be advantageous
    • Effective communication abilities are crucial for engaging with patients and community healthcare personnel
    • Possess a Valid Driver's License and current Auto Insurance
