    Description

    JOB SUMMARY: Reporting to the Manager of Acute Care Services independently implements the nursing process to provide safe and effective patient care throughout selected/assigned hospital nursing departments ("departments"). He/she demonstrates the knowledge and skills necessary to provide individualized, age-appropriate care to patients served in these departments. Provides clinical leadership, educational support, policy development and implementation of evidence based practices in support of Acute Care Services within the hospital.

    Will be directed, or autonomously go to, any department for which s/he is trained and competent, at any given time to provide nursing support, including break relief. Maintain the required proof of competency to ensure they are performing at the expected level of patient care.

    PERFORMANCE DIMENSIONS AND TASKS

    Essential Function

    1. Assesses the needs of patients and documents the identified needs in the patient record.
    a. Interviews the patient or other significant source and completes appropriate history information in medical record.
    b. Performs physical, psychological, sociocultural, spiritual, economic, and life-style factor assessments and documents findings.
    c. Utilizes multiple information sources including family, significant others, and healthcare team members in the assessment.
    2. Formulates diagnosis(es) appropriate to the patient's healthcare needs.
    3. Develops a plan for the care of the patient and the desired outcomes to be achieved.
    4. Implements care for patients guided by the plan of care.
    a. Sets priorities for patient care.
    b. Safely performs and documents the nursing procedures required by the patient assessment consistent with evidence-based standards and MMHS policy.
    c. Demonstrates proficiency in performing nursing skills required by patient population
    d. Demonstrates safe and efficient use of specialized equipment used in the clinical area.
    e. Demonstrates safe, accurate administration of medications used in the clinical area.
    f. Prioritizes own workload relative to patient care and department needs.
    g. Completes clinical assignments in a timely fashion including maintenance of equipment and supplies necessary for provision of patient care through the next shift.

    • Medication stock

    • Maintains safe and therapeutic physical environment.
    h. Completes non-clinical assignments, tasks, and projects within a pre-established agreed upon time frame.
    i. Coordinates patient care activities with other healthcare team members.
    j. Utilizes support resources as required.
    k. Communicates and interacts with patient/family in a way that is perceived as positive and supportive.
    5. Evaluates effectiveness of plan of care and modifies plan as necessary to achieve defined goals.
    6. Provides an environment that facilitates growth of the healthcare team.
    a. Uses both formal and informal teaching activities to provide information to healthcare team members.
    b. Assists leadership in identifying staff learning needs.

    Essential Functions

    7. Policies and procedures.
    a. Is knowledgeable of, follows and directs others to follow MMHS policies and procedures and any department specific protocols.
    b. Follows Infection Control policies/procedures, i.e., prescribed hand-washing and universal or transmission based precautions.
    c. Maintains cost containment efforts by prudent use of supplies and equipment; charging supplies and equipment appropriately.

    8. Problem solving
    a. Utilizes the steps of problem solving to resolve identified patient or staff related problems.
    b. Reports identified problems using the incident reporting process and chain of command.
    c. If available, responds cooperatively to requests for assistance to other departments.

    9. Utilizes appropriate professional communication skills.
    a. Listens to the ideas, opinions and feeling of others
    b. Uses communication strategies with patients, families and staff that result in:

    • Achievement of intended outcomes

    • Fostering a healing and supportive environment.

    10. Maintains professional responsibilities to enhance personal growth
    a. Understands and functions within the scope of the California Nurse Practice Act.
    b. Keeps all licenses and certificates current.
    c. Plans for personal change and growth based on evaluation and feedback.
    d. Participates in setting measurable goals for performance.
    e. Complies with annual skills update and any mandatory education by the due date assigned.
    f. Attends and participates in 80% of staff meetings per year and obtains information provided if unable to attend.
    g. Participates in committee work as requested.
    h. Reviews email and written communication during each shift worked.

    11. Supports the implementation of professional nursing practice within Acute Care Services.
    a. Acts as a clinical resource and provides clinical leadership to support the provision of patient care.
    b. Participates in the development of educational tools and resources to support patient care practices and improve care provided.
    c. Provides education opportunities as indicated in formal, informal, online and hybrid formats.
    d. Develops or assists in the development of clinical policies and procedures based on current research and guidelines.
    e. Participates in or leads the implementation of evidence-based practice projects.
    f. Participates in the department's performance improvement and quality initiatives throughout all project phases.
    g. Assists with department recordkeeping as directed by the manager.
    h. Facilitates department patient care assignments and assists in staffing projections.
    i. Participates in department charge reconciliation and serves as a resource to staff regarding department charges.
    j. Participates in the evaluation of products and supplies needed for patient care.

    12. Performs other duties as assigned.
    a. Performs other duties within capabilities as assigned by Manager.
    b. Willingly floats and cross-trains to other units as needed.
    c. Serves as a preceptor to new and temporary staff as requested by leadership.
