    As a part of our team, you will play a crucial role in our Hospice Care Center, managing admissions, working with medical professionals, and ensuring quality care for our patients.

    Key Responsibilities Include:

    • Tracking patient admissions and collaborating with medical teams.
    • Documenting assessments and developing care plans.
    • Ensuring regulatory compliance for inpatient admissions.
    • Working within the Interdisciplinary Team to coordinate patient care.

    Requirements:

    • Graduate of an approved school of nursing (AD, Diploma, or BSN).
    • Current license to practice nursing in North Carolina/South Carolina.
    • Preferably, experience in hospice or home care.
