Through its critical services, the organization delivers nearly 2 million meals to 20,000 at-risk older adults each year throughout Central and North Orange County.

JOB SUMMARY**:

Under the supervision of the Senior Case Manager, the Case Manager provides a coordinated and customized care plan for frail, at-risk elderly (60 and over) seniors within the Orange County service area.

The care plan meets the individual's needs by providing referrals and services that enhance client functioning and assists the client in living independently in their home.

Caseload for a FTE Case Manager will be at a minimum of 125 Cases.

In the event caseload decreases, SeniorServ management can modify the Case Manager schedule to meet the needs of the department.


ESSENTIAL DUTIES AND RESPONSIBILITIES:


  • Conducts clientspecific assessment of needs; identify problems and establishes client centered immediate requirements and longrange goals.
  • Arranges and coordinates a network of supportive services and entitlements (formal and informal) consistent with mutually developed care plan.
  • Monitors and evaluates effects of services in meeting longrange goals.
  • Budgets allocated monies and maintains records for homemaking and personal care services.
  • Arranges and coordinates homemaking and personal care services through contracted vendors.
  • Arranges and coordinates volunteer "chore" service through individuals or groups.
  • Maintains required records and reports in compliance with department, agency, local, State and Federal requirements.
  • Assists in developing community resources and maintain a current directory of services pertinent to the needs of the senior population.
  • Represents the agency at county and agency meetings at the discretion of the Department Manager.
  • Participates in scheduled team meetings to review client concerns/issues/goals.
  • Screens incoming homebound meal referrals for potential Case Management clients.
  • Participates in outreach efforts.
  • Practices safe operating procedures at all times and take necessary steps to help ensure a safe work environment.
  • Performs other duties as assigned relevant to Department functions.

EDUCATION, EXPERIENCE AND QUALIFICATIONS:


  • Bachelor's Degree in Human Services, Sociology, Psychology, social work or other related social services field of study.
  • Prior work experience with frail, elderly persons desired.
  • Sensitivity to and awareness of the special needs and resources for this population.
  • Keen assessment and evaluation skills with a thorough knowledge of case management principles and techniques.
  • Ability to maintain professional and confidential standards in client businessrelated activities.
  • Excellent communication skills, both verbal and in writing, required.
  • Ability to develop cooperative working relationships with community resources, clients, informal support sources and other employees.
  • Working knowledge of programs and services available in Orange County for seniors.
  • Must have own transportation, valid California Driver's License and auto insurance as required by state law.
  • Must pass postoffer background check.
