    Job Position: Registered Nurse Case Manager (RN) Bilingual

    Location: Chicago, IL 60628

    Pay Rate: starting at $30.00/hour (based on experience).

    Job Position: Bilingual RN Case Manager

    Schedule:M-F 8 AM to 5:30 PM. In-Office and meeting members face to face at designated office locations.

    Position Purpose:Perform care management duties to assess, plan and coordinate all aspects of medical and supporting services .

    Job Duties:
    • Develop, assess and adjust, as necessary, the care plan and promote desired outcome
    • Assess member's current health status, review past and present treatment plan and services
    • Coordinate services between Primary Care Physician (PCP), specialists, medical providers, etc.
    • Develop plan of care based upon assessment with specific objectives, goals and interventions
    • Facilitate member access to community based services
    • Monitor referrals made to community based organizations, medical care and other services
    • Enter and maintain assessments, authorizations, and pertinent clinical information into various medical management systems
    Education/Experience:
    • Graduate from an Accredited School of Nursing. Bachelor's degree in Nursing required.
    • Bilingual (required): Ability to read, write, and speak proficiently using the both English and Spanish language.
    • 2+ years clinical nursing experience in a clinical, acute care, or community setting. Knowledge of healthcare and managed care preferred.

    • Prior experience working in a remote role is preferred.
    • Prior case management experience preferred.
    Licenses/Certifications:
    • Current state's RN license (IL) both in active and good standing.
