We are seeking a dedicated and experienced Wound Care Nurse Practitioner to join our healthcare team.

As a vital member of our team, you will provide comprehensive wound care management to patients, utilizing your expertise in assessing, diagnosing, and treating acute and chronic wounds.

Responsibilities:

• Conduct thorough assessments of patients' wounds, including wound measurements, tissue assessment, and identification of underlying conditions.
• Develop and implement individualized treatment plans tailored to patients' specific wound care needs, including debridement, dressings, and adjunctive therapies.
• Collaborate with interdisciplinary healthcare team members to ensure comprehensive patient care and treatment outcomes.
• Educate patients and caregivers on proper wound care techniques, prevention strategies, and lifestyle modifications.
• Monitor patients' progress and adjust treatment plans accordingly to promote optimal healing outcomes.
• Maintain accurate and uptodate documentation of patient assessments, interventions, and progress notes.

Requirements:

• Current certification as a Nurse Practitioner.
• Master's degree in Nursing from an accredited institution.
• Active state licensure as a Nurse Practitioner.
• Minimum of 2 years of experience in wound care management, with proficiency in wound assessment, treatment modalities, and evidencebased practice.
• Strong interpersonal and communication skills, with the ability to effectively collaborate with patients, families, and healthcare team members.
• Commitment to providing compassionate, patientcentered care and promoting positive patient outcomes.

Preferred Qualifications:

• Certification in wound care (CWCA, CWCN, or equivalent).
• Experience working in an outpatient wound care clinic or hospital setting.
• Proficiency in electronic medical record (EMR) documentation systems.
