Delta Community Supports, Inc. is seeking a strategic and compassionate Licensed Practical Nurse to join our dynamic team.
Reporting to the Registered Nurse, the LPN will provide care, coordination and support to participants within the Intellectual Disability/Division of Developmental Disabilities (DDD) service programs.
Key Responsibilities:
Coordinate medical supports planning for new and existing support recipients.
Facilitate medical implementation of supports on an as needed basis.
Monitor medical supports for consistency with person-centered planning as well as the company's mission, values and Individual Supports Policy and Procedures.
Administer and record medications that require medical personnel on an as needed basis.Provide staff with training and resources in health care issues such as but not limited to seizures, hepatitis, diabetes, recognizing signs of illness and special diets.
Establish Medical Support Guidelines for individual support recipients as necessary and to instruct staff on the implementation of such guidelines.
Update and publish special precautions lists for individuals with seizure disorders, diet restrictions, allergies, and other illnesses on an as needed basis.
Maintain liaison with state and regulatory personnel, families, and external sources of support and communicate with consulting psychiatrists, psychologists, and healthcare professionals as needed.
Qualifications:
Graduate from an accredited school of nursing program
Current, unrestricted license to practice as a Licensed Practical Nurse in the state of assignment.
Two (2) years of experience providing community based supports (residential)
Valid driver's license in the state of residence.
