    * JOB DESCRIPTION

    Provide case management and behavioral health screening services for identified DCRP participants to assist with their reentry from incarceration. Services. This is an in-person, field-based position which will also include work within the Delaware County jail. This position will be part of a team working in collaboration with County and community-based providers in Delaware County.

    RESPONSIBILITIES :

    · Provide case management and behavioral health screenings to individuals who are incarcerated and who have reentered into the community in Delaware County.

    · Documentation of service provision within 24 hours of service completion in PHMC's electronic health record.

    · Case management services include: resource coordination while incarcerated and post incarceration support monitoring of adherence to follow-up with service referrals including attendance at behavioral health treatment programs

    · Behavioral health and screening assessments for individuals within 30 days of release from incarceration.

    · Travel throughout Delaware County meeting with programs' participants and providers.

    · Active participation in team meetings, scheduled case conferences, trainings as assigned.

    · Active participation in individual supervisions case reviews, and scheduled team building events as assigned.

    · Collaboration with Delaware County jail personnel, probation/parole, community-based providers and other assigned agencies as identified.

    · Assist individuals to link to reentry support services faith-based institutions, behavioral health services, housing and employment resources, and other identified community supports

    · Maintain participant records in accordance with agency standards, managed care, and licensing requirements

    · Prioritize assisting participants to enroll in behavioral health, physical health services, housing applications, employment, entitlements, vocational services, and educational services.

    · Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach.

    · Majority of work is performed week in the community, correctional facility and other locations where participants are available/accessible for meeting

    · Flexible work schedules if needed to provide participant services during non-traditional business hours

    · Weekly CM/Evaluator and participant meetings while incarcerated and biweekly post incarceration.

    · Identify community resources to support participants' reentry efforts and make referrals as appropriate.

    · Coordinate activities with other members of the DCRP including public defender, district attorney, probation department

    · Participate in DCRP meetings, Forensic Service meetings and trainings as requested.

    * SKILLS :

    · Must have knowledge of behavioral health services and Delaware County social service programs.

    · Excellent public speaking skills

    · Excellent interpersonal skills

    · Ability to meet timelines

    · Excellent oral and written communication skills.

    · Good analytical and problem solving skills.

    · Working knowledge of forensic population

    * EXPERIENCE :

    · Experience in behavioral health services.

    · Minimum of two to three years of professional experiences working within the criminal justice system.

    · Experiences in providing case management/evaluation services to chemically dependent forensic population a plus

    · Bi-lingual plus.

    * EDUCATION :

    · Bachelor's degree in related field including psychology, social work, education and two (2) years of direct participant contact and/or drug and alcohol setting, preferred.
