The Hybrid Transition and Community Support Team will assist Class Members in transitioning from nursing facilities to the community and provide support services to Class Members post-transition in the community.


The Team Leader will oversee the hybrid transition and community support team that is responsible for supporting the Comprehensive Class Member Transition Program in achieving program goals.

They will provide direct clinical supervision of team members and manage employee workflow ensuring the transition process from nursing home to independent living in the community for class members.

They will run team meetings; help develop and monitor treatment plans for the team's clients; and develop and administer tracking systems for required documentation to ensure quality service and effective intervention are delivered to clients.

The Team Leader will routinely interface with organization leaders to enhance communication, cooperation/collaboration across programs/departments, and to improve the quality and comprehensiveness of services provided.

The Team Leader will provide and ensure the team provides client-centered, strengths-based, and trauma-informed services to clients with severe mental illness and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach.


Responsibilities:


  • Serve as the senior clinician on the team and provide coaching and mentoring to team members.
  • Facilitate team meetings; and assist with onboarding and ongoing training to provide quality services to clients.
  • Provide individual and group supervision to team members to problem solve, train, monitor, and guide their clinical case management and service delivery responsibilities.
  • Assist in administrative processes which include performance reviews, disciplinary actions, interviewing, and hiring decisions.
  • Monitor employees' direct service hours and ensure that direct reports and the team meet their direct service targets.
  • Assist the team with coordinating schedules to ensure all clients that need to be seen are on the schedule.
  • Assist the team in prioritizing transition activity to maximize the goal of moving class members into the community.
  • Assist in all areas of the Comprehensive Class Member Transition Program; i.e. community integration, skill building, transition preparedness, and overall recovery support in the community.
  • Work with teams to regularly assess the appropriate level of care and engagement of clients. Implement transition/discharge plans as needed.
  • Meet with clients directly to provide services related to individual goals.
  • Uphold standards and guidelines of the Consent Decree Programs by ensuring employees follow through with submission of required documents and services.
  • Ensure that accurate and detailed mental health assessments/other documentation are complete based on required timelines (i.e. IM CANS, LOCUS, NOM's, Service Plans) as required.
  • Demonstrate understanding of collaborative and concurrent documentation processes, and strives towards this as a best practice.
  • Lead the team and demonstrate good judgement in responding to crisis situations as they occur and minimize risk to clients and employees.
  • Model effective communication within the team model for a multidisciplinary approach to client care.
  • Coordinate care with interdisciplinary employees and family members to ensure continuity of care for clients.
  • Assist with building positive rapport and effective communication with nursing facility employees, managed care organizations, subsidy providers, UIC-College of Nursing, Illinois Department of Human Services, and other community partners to support comprehensive quality care.
  • Assess team's morale and continue to work towards keeping employee and team culture positive.
  • Assist in program and organizational trainings as needed.
  • Perform other related duties and/or projects as assigned.

Qualifications:

  • Must have one of the following requirements:
  • A licensed social worker (LSW) possessing at least a master's degree in social work with specialized training in mental health services or with at least 2 years' experience in mental health services
  • A licensed professional counselor possessing at least a master's degree with specialized training in mental health services or with at least two years' experience in mental health services
  • A registered nurse (RN) with at least one year of clinical experience in a mental health setting or who possesses a master's degree in psychiatric nursing
  • An occupational therapist (OT) licensed under the Illinois Occupational Therapy Practice Act [225 ILCS 75] with at least one year of clinical experience in a mental health setting
  • An individual possessing at least a master's degree in counseling and guidance, rehabilitation counseling, social work, vocational counseling, psychology, pastoral couns
