NVCS Case Management Team Expectations


Kids First is a project established to improve educational outcomes of housing insecure students in the Los Angeles Unified School District (LAUSD).

The project seeks to improve educational outcomes through a partnership between LAUSD, North Valley Caring Services, and New Economics for Women in which collaborative case management and wrap around services including basic needs, mental health resources, financial wellness coaching, and LAUSD PSA counseling are utilized to break barriers that impede academic success for families.

Central to this project and its theory of change, is the case management practice and intensive collaboration.

As a member of the case management Team, case managers from each agency are expected to serve as 1) Case Managers and 2) Liaisons to their Agency as outlined below:


Case Management:


  • Schedule and conduct introductory meeting and basic needs assessments for each enrolled family.
  • Carry a caseload of about 1525 families and ensure that they are connected to services and handed off to other agencies as needed.
  • Attend weekly case conference meetings and come prepared to share updates and discuss progress.
  • Meet with the families on the caseload every 1 2 weeks for a general check and rapport building.
  • Connect clients to appropriate outside agencies for needs not provided by the program (i.e. DPSS)
  • Work with clients and case management team to develop SMART goals and carry out interventions to improve educational outcomes.
  • Report progress, concerns, and ask for assistance with clients during weekly case conference.
  • Evaluate client progress, discuss with the team, and adjust to improve outcomes.
  • Maintain consistent and detailed notes on each interaction and service delivered to the client.

NVCS Basic Needs Liaison:


  • Keep up to date on all the resources that NVCS offers and the process to enroll/utilize the services.
  • Connect clients in Kids First to services at NVCS as the need for a service is identified via referrals from other Kids First case managers, selfreferral from clients, or as detected by you upon hearing about a client's situation.
  • Provide all Kids First clients as needed with basic needs delivery through emergency meal delivery, boutique vouchers, and enrollment into summer classroom/afterschool program.

Job Types:
Full-time, Contract


Pay:
$52, $60,000.00 per year

Experience:

Social Work: 4 years (required)
