Program Coordinator

35 hours per week / $ $31.00 per hour
(The actual compensation will be determined based on experience and other factors permitted by law.)


Supervision Responsibilities:

Under the guidance of the Employment Services Program Manager, will assist with the organization and direction of the Employment Service Coordinators and other assigned staff.

Provides leadership responsibilities for team members during program or service activities and provide secondary on-call services based on Department Of Rehabilitation and allied services program.


Summary:

Under the general supervision of the Program Manager the Program Coordinator will assist adults with mental illness to improve and enhance their level of employment functioning and to identifying employment interests, skills, experience and options in order to facilitate the member's development and pursuit of a plan that leads to employment in the community based on the employment rehabilitation model of treatment and services.


Essential Duties and Responsibilities:

  • Oversees and ensures programs monthly, quarterly, and yearly data reporting. With accurate and timely submission and documentation.
  • Actively provides support to individuals on the team as directed and requested by the Program Director or Program Manager.
  • Provide staff support in linking members to activities to meet their goals/needs.
  • Assists with the leadership of the daytoday operations of the department as needed. Participates as an active member for quality improvement, safety committee, domains or other leadership teams as needed or assigned.
  • Provides timely information and feedback to Program Manager and Program Director with regard to emergent staff needs, team or member issues and work with executive management to provide intervention to redirect staff and/or programs when objectives are not being met.
  • Participate as an active member of the site management team, quality improvement, and other designated leadership team as needed and assigned.
  • Read perform researches, attend classes, workshops, Relias trainings or seminars and participate in other professional development activities in order to maintain and enhance personal and professional skills.
  • Complete all documentation needed through CARELOGIC and other data systems.
  • Track community placements, work adjustment, adult work experience and subsequent support services.
  • Oversees feedback loop to DOR counselors.
  • Evaluate and provide input regarding improving service delivery to work adjustment and adult work experience members.
  • Assist with the implementation of Commission on Accreditation of Rehabilitation Facilities (CARF) standards thorough the agency as required, prepare for CARF survey and implement recommendations outlined in the survey reports. Maintain services, programs and related information flow consistent with CARF standards.
  • Support staff in designing and implementing activities that link members to work activities that meet their goals and needs.
