    Job Duties:

    • Assessment of patients, vital signs, complete review of medical history, medications, interventions, and care plan needs/changes.
    • Strong attention to detail.
    • Med administration, EMR charting. NG tubes, blood product admin.
    • Coordination and communication with care team and patient/family.
    • Referrals, prior auth of procedures and medications, communicating with insurance team.
    • IV/wound care/phlebotomy/care plans/refills/scheduling/injections.
    • Provides for patient comfort, safety, and protects dignity, privacy and confidentiality. (Adherence to Policy)
    • Improved patient care by implementing evidence-based practices and collaborating with multidisciplinary teams.

    Job Requirements:

    • 2 years of Skilled Long Term Care/Transitional Care Experience.

    • LPN License - Required.
    • BLS - Required.

    Schedule:

    • 8 hours shift
    • Night shift
    • Every Other Weekend

    Job Type:
    Contract


    Pay:
    $46.00 per hour


    Physical setting:

    • Long term care

    Standard shift:

    • Night shift

    Weekly schedule:

    • Rotating weekends

    Experience:

    • Skilled Long Term Care/
    Transitional Care: 2 years (Required)


    License/Certification:


    • LPN License (Required)
    • BLS Certification (Required)
