Case Manager

Responsibilities:
- Provide case management services to clients, including assessment, planning, coordination, monitoring, and evaluation of services.
- Develop and implement individualized service plans for clients.
- Provide crisis intervention and support services.
- Provide advocacy and support to clients.
- Maintain accurate and uptodate client records.
- Collaborate with other professionals to coordinate services for clients.
- Provide referrals to community resources.
- Participate in team meetings and other professional development activities.
Qualifications:
- Minimum of one years of experience in case management.
