  • Develop and implement individualized service plans for clients.
  • Provide crisis intervention and support services.
  • Provide advocacy and support to clients.
  • Maintain accurate and uptodate client records.
  • Collaborate with other professionals to coordinate services for clients.
  • Provide referrals to community resources.
  • Participate in team meetings and other professional development activities.

Qualifications:


  • Minimum of one years of experience in case management.
