    POSITION: Home Visitor

    DEPARTMENT: Early Childhood

    LOCATION: Lower East Side and Brooklyn

    REPORTS TO: Early Childhood Director

    SALARY: $30,940-35,800

    SCHEDULE: Monday- Friday

    JOB SUMMARY: Provide high-quality, culturally competent child development and parent support services with an emphasis on the role of the parent as the child's first and most important relationship. The Home Visitor will also assist with the completion of the child health requirements and other required screenings and assessments while supporting the larger system of Family and Health/ Nutrition Services.

    EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS:
    • High School Diploma and Home- Based(home visitor) Child Development Associate (CDA) credential or comparable credential, or equivalent coursework as part of an associate's or bachelor's degree college credits in Social Work, Human Development or Early Childhood Education preferred)
    • Experience working with young children and understanding of principles of adult learning
    • Experience working with and knowledge of community resources
    • Ability to effectively communicate orally and in writing with family, staff and community members
    • Ability to foster and maintain positive, professional relationships with participating families
    • Ability to use public transportation and travel independently
    • Must be able to work independently
    • Excellent computer skills including strong working knowledge of Microsoft Word and Excel
    • Bilingual English/ Chinese or Spanish strongly preferred
    PRE- EMPLOYMENT REQUIREMENTS:
    • Successfully clear; NYS DOI Fingerprint Screening, NYS Central Registry Clearance, Sex Offender Registry Clearance, three (3) reference checks, physical examination with updated TB test, Tdap, MMR, varicella (fees may apply).
    • Meet all employee health requirements in Article 47 (DOHMH) and Head Start performance standards.
    • Mandated Reporter training certificate (every 2 years) and comply with agency's policy and procedure regarding identification and reporting of child abuse and neglect.
    • Obtain First Aid/CPR and Preventive infectious diseases certificate within the first year of employment.
    • If hired, Grand Street recommends all potential employees to have COVID-19 vaccination and booster.
    ESSENTIAL DUTIES AND RESPONSIBILITIES:

    Under the direct supervision of the Education and Disabilities Coordinator, the Home Visitor shall be responsible for:
    • Maintaining a caseload of (12) participants
    • Providing 90-minute weekly home visits to support parents/guardians in their roles as primary caregivers and to facilitate the child's individual development within their home environments
    • Providing regularly scheduled home visits with enrolled pregnant women to ensure access to comprehensive prenatal and postpartum care and to provide prenatal education on topics such as fetal development, labor and delivery, postpartum recovery (including maternal depression), and the benefits of breastfeeding.
    • Independently travel between locations in Manhattan and/or Brooklyn within walking distance and by use of public transportation
    • Collaborate with the Education Coordinator and other staff members to plan and implement developmentally appropriate parent/child focused Group Socializations
    • Assisting parents in identifying strengths and goals leading to school readiness
    • Practice positive teacher- child relationships including but not limited to working and speaking with children face- to- face at their level
    • Working in collaboration with families to identify goals for themselves, their families and their child, and developing objectives for meeting those goals for the Family Partnership Agreement
    • Collaborate with families and team members on using Home Based curriculum and parenting curriculum resources
    • Complete ongoing assessment of children's development in keeping with identified EHS and GSS school readiness goals. Utilize Home Based curriculum and TSG assessments.
    • Record Keeping and reporting to ensure compliance with Head Start regulations and high quality programming: Enters child and family information into the approved child tracking system database (COPA & DOE systems). Assist with collecting child health medicals and dentals along with entering health related data into COPA. Submits reports as needed and requested, such as monthly summary tracking reports, etc. Maintains and provides data for the annual Program Information Report (PIR). Records, monitors and follows up on referrals for support services for reporting requirements and ensures that all documentation is current in child's file and in compliance with Head Start/DOHMH regulations
    • Facilitating and supporting the participation of home-based families in program governance and program planning and assessment
    • Assisting families in accessing and utilizing community resources, arrange and keep medical and dental appointments required by EHS and DOH, including arranging transportation when necessary
    • Working collaboratively with supervisor and program specialists/consultants (health, nutrition, mental health, disabilities and education) to ensure achievement of family goals, school readiness, and program compliance
    • Participate in monthly supervision meetings, case conferences and mentoring sessions
    • Partnering with GSS Community Hub staff and Single Stop in coordinating referral services including prenatal care and implementing parent resource groups and trainings
    • Active, ongoing participation in program recruitment and intake efforts
    • Follow applicable state regulations and agency policies regarding the prevention and reporting of child maltreatment.
    • Participate in ongoing staff meetings and professional development trainings
    • Maintaining flexible schedule, including some evenings and/ or Saturdays
    • Being able to lift 35 pounds
    • Other duties as assigned
