The Case Management department is comprised of nurse care managers and social workers. We touch the lives of patients in the emergency department and inpatient medical floors. Our main goal is to discharge patients to the next most appropriate level of care. We strive to provide individualized discharge plans formulated between the patient/family, multidisciplinary team, and community. Every day is different with a new challenge, so you will not become bored
