    Work Schedule:

    Monday to Friday, either 8:00 AM to 4:30 PM or as needed.

    Job Summary:

    As the Care Manager for Homeless Services, your role involves providing advocacy, coordination, and evidence-based care for individuals and families in need. Collaborate with triage, assessment, planning, and communication within the community services.

    Job Responsibilities:

    • Conduct thorough assessments and coordinate community resources based on patient needs
    • Ensure timely completion of all clinical and administrative documentation
    • Maintain cooperative relationships with internal and external resources
    • Develop service plans with clear goals and objectives
    • Manage workflow efficiently

    Job Requirements:

    • Qualifications: Bachelor's degree in Human Services or related disciplines
    • Must be at least 21 years old with a valid driver's license and reliable transportation
    • Ability to analyze clinical records, write reports, and communicate effectively
