Position Summary:
Become an integral part of our team dedicated to serving the Olney Region (13 surrounding counties) by delivering quality care to patients of all ages and diagnoses.
* Nurse to Patient ratio: 5-6 patients daily, adjusting as necessary based on travel for patient care.
* Collaborate closely with various departments to ensure comprehensive care.
* Specialize in assisting patients requiring wound vacs, various IV pumps, pleural drains, and more. We are seeking a compassionate, skilled, and reliable home care nurse to strengthen our team.

Take on the responsibility of coordinating and providing both direct and indirect patient care utilizing the nursing process to address the physical, psychosocial, environmental, and spiritual needs of specific patient populations and families within the geographical area of Carle Home Services.
To excel in this position, strong interpersonal skills, effective time management, the ability to build connections with patients, and deliver superior healthcare with minimal supervision are essential qualities.
Qualifications:
EDUCATIONAL REQUIREMENTS
Associate's Degree in Nursing CERTIFICATION & LICENSURE REQUIREMENTS
Registered Professional Nurse (RN) License Illinois upon hire and Basic Life Support (BLS) within 30 days and Driver's License upon hire and Proof of Auto Insurance upon hire.
EXPERIENCE REQUIREMENTSOne (1) Healthcare/Medical - Nursing General ADDITIONAL REQUIREMENTS Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients SKILLS AND KNOWLEDGE
Ability to work as a member of interdisciplinary team. Drives reliable transportation and maintains state required insurance and Illinois Drivers license in good standing. Current CPR.
Essential Functions:
Demonstrates understanding of Medicare Home Care benefits including covered services, recertifications, transfers, and non-recertifications. Follows regulatory requirements regarding documentation standards.
Comprehends department specific indicators.
Carries out case management activities involving assessment, coordination, planning, monitoring, implementation, and evaluation. Engages with clients, caregivers, and families to assess needs, plan care, coordinate services, monitor progress, and provide support and education.
Ensures timely completion of all admission documentation in accordance with policy guidelines.
Submits completed admission documents promptly as per policy requirements.
Updates OASIS/485 changes within 24 hours of receipt.
Records and transmits visit notes and phone conversations in a timely manner (including physician orders, admission, routine, supervisory, telephone, and discharge notes).
Ensures timely writing and transmission of Physician Orders, with immediate completion of corrections to care plans per established timelines.
Reviews and verifies verbal orders with MD office, ensuring written confirmation is processed immediately and transmitted within policy timelines.
Once documented and transmitted, orders are final. Includes patient visit details in records to prevent omission of visits.Clearly marks the correct acronym on all physician orders.
Conducts Home Health Aide supervisory visits.
Serves as the coordinator within the healthcare team to maintain seamless care transitions.
Initiates case conferences when required and documents all interactions (with other disciplines, MD, care coordinator, insurers, etc.). Shares relevant information for patient care conferences on all admissions allocated.Practices in a manner that respects the needs of patients and families, delivering care in line with established plans and orders.
Supervises the activities of licensed practical nurses.
Assigns tasks to home health aides, provides written instructions, and supervises their activities within patient homes.
Demonstrates proficient knowledge of State and Federal regulations and accreditation standards through daily practice.Conducts OASIS assessments for admissions, recertifications, resumptions, and discharges.
Administers advanced wound care procedures such as wound vacs, packing, staple or suture removal.
Monitors central lines including PICC and Hickman catheters, undertaking dressing changes as per agency guidelines.
Performs mediport accesses for IV infusions and blood draws, adhering to agency protocols.
Manages case requirements for assigned patients effectively.
Communicates patient updates and condition changes promptly with MDs via phone or electronic channels.
