Job Summary
Manage a designated caseload to coordinate and complete timely assessment, planning, implementation, and evaluation of discharge plans and care transitions. Ensure optimal utilization of resources and comply with regulatory requirements.
Key Responsibilities
Requirements
Education: BSN or MSW required
Experience: 3 years of relevant experience
Certifications: Case Management Certification required within 3 years of hire
Licensure: Current or compact RN licensure in the state of NC (if BSN)
