Caseworker Job Description


Job Summary:


The Caseworker at Road to Renewal plays a critical role in ensuring the successful and secure reintegration of individuals into the community.

This position requires a detail-oriented and diligent professional to verify various aspects of residents' plans, including home arrangements, potential visiting locations, employment, and daily check-ins, contributing to the overall safety and success of our reentry program.


Responsibilities:


  • Supervise and monitor the health and safety of residents in the program.
  • Completion of daily monitoring and communication log.
  • Assisting residents in following daily schedule and routines.
  • Assist with resident needs and daily requirements.
  • Assist with daily operations such as food shopping, meal preparation, monitoring of chores, household tasks as assigned.
  • Responsible for home and visiting location verification and make sure residents are visiting only safe places and check in on residents at work.
  • Thoroughly check and verify residents' proposed home plans to ensure they align with program requirements and community standards.
  • Verify potential visiting locations to ensure a safe and supportive environment for residents.
  • Provide support in navigating with community relationships
  • Set expectations with friends and family
  • Conduct comprehensive employment verification, collaborating with employers to confirm residents' employment status and work arrangements.
  • Establish and maintain a daily checkin system, contacting residents at predetermined times to monitor their wellbeing and adherence to program guidelines.
  • Maintain accurate and uptodate records of verification processes, documenting findings, and any deviations from the approved plans.
  • Work closely with the Case Managers, Assistant Program Director, and Program Director to ensure alignment with overall program goals and standards.
  • Must be able and ready to respond to any verificationrelated crises promptly, collaborating with relevant parties to ensure residents' safety.
  • Responsible for maintaining the staffing ratio as assigned by the Program Director; (remaining onsite as directed by supervisor).
  • Provide immediate assistance and support during crisis situations, including emotional distress or aggression, and implement appropriate interventions.
  • Demonstrate cultural competence and sensitivity in all interactions, recognizing and respecting the diverse backgrounds and experiences of clients.
  • Work collaboratively with other members of the reentry program team, including case managers, Assistant Program Director, and external service providers, to ensure a comprehensive and holistic approach to client support.
  • Adhere to all Programmatic Policies and Procedures, including Driving Policy.
  • Occasional transportation of residents is possible.
  • Adhere to LHI employee handbook rules.
  • Appropriate utilization of oncall procedures.
  • Responsible for the immediate notification of safety/health/legal issues happening in the program (resident or programmatic), including completing an Internal or Critical Incident Report form.
  • Complete medication training with PD
  • Observe and document residents' selfadministration of medications.
  • Complete daily medication counts.
  • Other duties as assigned.

Compliance and Reporting:


  • Adhere to any company and program policies, procedures, and legal and ethical standards.
  • Strictly adhere to State and Federal HIPAA and PREA guidelines.
  • Follow mandated reporting guidelines.

Qualifications:


  • Bachelor's degree in a relevant field or equivalent work experience preferred. High School Diploma or GED required.
  • Previous experience in verification processes, preferably in a reentry or community support setting preferred.
  • Detailoriented with strong organizational and documentation skills.
  • Ability to communicate effectively and professionally with residents, employers, and other stakeholders.
  • Understanding of the importance of maintaining confidentiality and adhering to ethical standards.
  • Knowledge of substance abuse treatment, mental health, and traumainformed care principles.
