Job summary
The Nutritionist/Dietitian Provides individualized nutrition counseling and diabetes self-management education/training to individuals and groups. Supports patients in making sustainable lifestyle changes to improve overall health outcomes with special focus on the diabetic population at Gonzaba Medical Group clinics.- Provides nutrition assessment counseling and individualized meal planning tailored to medical conditions cultural preferences and lifestyle.
- Offers diabetes self-management education through both group classes and one-on-one sessions.
- Collaboratively develops goals learning objectives and educational plans with patients.
