    Under the general direction of the Public Health Supervisor, the Registered Nurse represents a variety of positions responsible for providing skilled nursing care and services in various public health program areas (e.g. family health, care coordination, etc.); assessing and evaluating the health of clients; formulating and developing health care plans; providing health education, consultation and teaching to clients; providing screening and referral services; maintaining and documenting health records, services, case management records in accordance with specific program guidelines, regulations and department policies and procedures.

    This position primarily works as the South Country Community Connector.
    Perform the duties of the South Country Community Connector Care Coordination of South Country Health Alliance PMAP population. Care Coordination/Case Management of the Nursing Home population. Actively participate in Community Health Assessment activities
    A Community Care Connector is the local South Country Health Alliance expert in each of our service area counties. Working either in the county public health or human service department, the Connector works to bridge the gap between medical services, public health, human services and other community resources. They work collaboratively with South Country staff to provide support at the local level.

    Duties/Responsibilities
    • Assesses and evaluates the health status of individuals and/or families in their home, community or specific sites (e.g. public health office, schools, etc.).
    • Understand MCO policy/procedure and covered services.
    • Assesses and evaluates the health status and needs of individuals or families in the home or community. Develops health plans or plans of care to address individual, family or community needs.
    1. Performs comprehensive assessments examining demographic data, physical conditions, health history, medications, mental health status, physical environment, social/family dynamics, general observations and the like.
    2. Identifies risk factors and vulnerabilities.
    3. Conducts interviews and formulates individualized care and health plans.
    4. Identifies service goals, outlines problems, goals and interventions.
    5. Collaborates with other health partners to initiate and/or implement plans of care.
    • Performance measures/quality improvement in program areas.
    • Provides skilled nursing care to individuals and/or families in accordance with the Nursing Practices Act.
    1. Performs delegated nursing medical functions and physician orders. Administers medications. Provides first aid.
    2. Clarifies and obtains orders from physicians to address health needs/issues.
    3. Provides crisis intervention, as needed.
    4. Provides referral services internal and external health care providers.
    5. Provides ongoing evaluation of individual/family needs or health issues.
    6. Provides nursing services in response to any emergency preparedness, disasters, health outbreaks/emergencies, etc.
    7. Carries out communicable disease, immunization requirements and infection control practices.
    8. Surveillance and monitoring of both reportable and non-reportable acute and chronic diseases at the individual, family and/or community level. Implements mandated reporting requirements, immunizations, TB surveillance, blood pressure control, STD's, and teen pregnancy.
    9. TB Case Management
    • Collaborate with CHS health Services (Meeker & McLeod Counties)
    • Monitor CHW care plans and sign off on referrals.
    • MnCHOICES assessor for over 65 population.
    • Documents and maintains all records, case records, forms, and reports in accordance with audit requirements, program requirements/regulations, and departmental procedures. Assists in preparing health service/status/activity reports, per department procedures (immunization reports, medication administration, client visits, care plan updates/changes, etc.).
    • Provides public health education services, counseling, teaching or consultation services for targeted populations, individuals, families or the community.
    1. Provides individual/family counseling and teaching to promote self care and wellness.
    2. Educates and provides interpretations to individuals and/or families regarding the implications of medical diagnoses and provides support in implementing treatment.
    3. Serves as an advocate for wellness and health programs. Addresses questions, concerns and issues as presented
    4. Provides health education and educates on health topics. Provides information to clients on nutritional issues.
    5. Provides consultation to jail personnel in jail inspection.
    • Performs other duties of a comparable level or type, as required.
    1. Keeps abreast of changing trends, policies, best practices and developments in areas of responsibility.
    2. Attends conferences, seminars, regional meetings and participates in various professional organizations, etc
    Minimum Requirements

    A valid MN Driver's license. MN Registered Nursing License; CPR and First Aid Certificate.

    DEGREE INFORMATION:
    Type of degree: (B.S., M.A., etc.) AA or BA/BS

    Major field of study or degree emphasis:Nursing

    No previous experience required beyond educational and certification requirements..

    Additional Information

    Skilled in:
    • Evaluating and performing client health assessments; screenings and care planning.
    • Writing individual health plans and health plans for at-risk clients in accordance with relevant rules and regulations and program policies and guidelines.
    • Interpreting and applying polices, rules, guidelines and procedures pertaining to the planning and delivery of health care services.
    • Evaluating, assessing and making nursing health care assessments.
    • Providing skilled nursing care, immunizations, and/or health testing for clients.
    • Basic math skills.
    • Operating and using computers and office productivity software (e.g. word processing, email, presentational and spreadsheet applications, etc.) in. documenting and maintaining health records, files, forms and reporting
    • Time management and group facilitation skills.
    • Presentational skills.
    • Educating and informing county residents, individuals and families concerning health issues, concerns, communicable disease prevention and control, nutrition, pregnancy, child development and infant care.
    • Virtual and in-person communication, writing and interpersonal skills as applied to interaction with coworkers, supervisor, the general public, etc. sufficient to persuade and/or influence others in matters relevant to department operations and health issues.
    • Flexibility in schedule for after business hours events.
    Essential knowledge and specialized subject knowledge required to perform the essential functions of the job:
    • Principles, practices, issues, trends and techniques in the field of public health and nursing.
    • Public health and nursing concepts, practices and trends related to public health; communicable diseases, their etiology, transmission, control, prevention and general epidemiology issues.
    • Knowledge of public health programs, services, and assessment criteria/tools.
    • Knowledge of nursing technical procedures, care, treatments and processes.
    • Knowledge of current trends in the delivery of health care services and effective delivery of Public Health Nursing Services.
    • Knowledge of emergency preparedness issues and processes within the County.
    • Legislative rules and regulations pertaining and impacting public health issues, services and program activities.
    • Community resources and related services within and outside of the County.
    Physical requirements associated with the position can be best summarized as follows:

    Medium Work:

    Exerting up to 50 pounds of force occasionally, up to 25 lbs frequently, and/or a negligible amount of force constantly to lift, carry, push, pull or otherwise move objects, including the human body.

    Unusual or hazardous working conditions related to performance of duties:

    Duties associated with the job involve travel and exposure to blood, body fluids, contagious diseases, sharps, public contact or other environmental risks requiring constant precautions and safety considerations when dealing with the public in their homes or in the community. Duties include responding to public health emergencies and being exposed to disease and contagious outbreaks. Risks and hazards can be minimized to some extent by risk management procedures, safety precautions/equipment, training and professional judgment.
