    Registered Nurse (RN) Case Manager

    Opportunities in: Fort Collins, Colorado

    Compensation: $30-$150 per visit (dependent on visit type), PLUS mileage and travel compensation with on-call requirement

    As a Home Health Registered Nurse, here's an overview of your responsibilities:

    • Deliver in-home care to patients dealing with illnesses, injuries, and chronic conditions, often post-discharge from medical facilities
    • Collaborate with a medical team, supervising LPNs, CNAs, and aides in patient care
    • Conduct patient assessments, monitor progress, and communicate changes to the attending physician
    • Educate patients on their care plan, including prescribed medication, therapy, diet, and exercise regimen
    • Provide guidance to family members on the patient's care plan, medication, and home safety
    • Ensure treatment goals are achieved and coordinate patient discharge

    Essential requirements for Home Health Registered Nurses:

    • Active Registered Nurse (RN) License in Colorado
    • Alumnus of an accredited nursing school or program
    • Minimum of 1200 hours of RN experience
    • Current Health care CPR/BLS certification (completion of a demonstration course is mandatory)
    • Preferably, prior experience in home health care
    • Strong organizational abilities
    • Capability to manage patients with chronic ailments
    • Successful completion of a background check and drug test (without THC)
    • No Covid Vaccination Mandate
