Position Summary:_

In accordance with California law, the expected salary range for this California position is between $16.00 and $18.00. The actual compensation will be determined based on experience and other factors permitted by law.

Position Summary:_


The Outreach Worker provides street-based crisis intervention services, gateway services, referrals, and crisis support counseling to homeless youth on the streets.

Supports homeless youth transition into a stable living environment.

Job Duties & Responsibilities_

  • The Outreach Worker will work out in the community, as well as spend time at Operation SafeHouse shelters to ensure a seamless transition for youth that are leaving the streets.
  • Work closely with all street youth who voluntarily enter into SafeHouse programs.
  • Conduct intense case management and provide wrap around services.
  • Provide continuous transportation throughout case management.
  • Enroll clients into Medical & CAL Fresh programs.
  • Assist clients in becoming document ready. (Assisting with IDs, Birth Certificates, and Social Security Cards.)
  • Developing an individualized "Wellness Plan" that encompasses housing, services, and wellness.
  • Connect clients to medical/ mental health resources as needed.
  • Complete daily time and activity sheets.
  • Complete all intake paperwork for youth who partake in SafeHouse outreach services.
  • Responsible for tracking the progress of youth who have entered housing programs. This includes followingup for up to 3 months once they have exited the program.
  • Conduct street outreach with other street outreach team members to ensure personal safety

For example:

Make weekly trips/visits to bus stations, local malls, Safe Place sites and community centers located at parks and other areas where street youth congregate.


  • Complete and submit monthly reports to Outreach Director by the 1st of each month. Maintain statistical data as assigned.
  • Maintain clean driving record, keep company vehicle clean and report maintenance issues/auto repair issues to agency in written form.
  • Attend community fairs to promote awareness of agency programs and interact with street youth to build trust in our services.
  • Maintain proper boundaries with clients to ensure appropriate relationships.
  • Complete applicable paperwork such as Apricot & VISPDAT and enter data into HMIS.
  • Other duties as assigned.

Skills and Qualifications:_


  • High School Diploma or equivalent experience.
  • Experience working with at risk youth or homeless population.
  • Good listening and problem-solving skills.
  • Have the ability to work as a team and strong networking skills to build community partnerships.
  • Excellent oral and written communication skills; Computer and word processing skills.
  • Must have current driver license, clean driving record and reliable transportation to provide services listed above.
  • Pass state criminal background check.

Physical Demands:_

  • Must be able to remain in a stationary position
  • Light work that includes moving objects up to 20 pounds.
  • The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc.
  • Constantly works in outdoor weather conditions.
  • Move from one location to another to provide program services to youth on streets.
