    The Integrated Recovery Program SUD Assessment & Care Coordinator works with individuals and families who have been screened for or identified as having a substance use disorder. The coordinator is responsible for completing the GPRA data collection tool to measure participant changes in substance use and identify the best supports to aid in their recovery and is accountable for the timely submission and follow-up of the GPRA.

    This position also works with other members of an interdisciplinary team to ensure participants have the support they need to be successful in their recovery from substance use or abuse. This position supports in the form of facilitating linkages to other programs, providing referrals for services internal and external to Mary's Center, as well as working with pharmacies and insurance companies to ensure participants have easy access to medication assisted treatment. The position requires an openness to and understanding of co-occurring disorders treatment, trauma-informed care, and a harm reduction approach.

    Reportability

    This position will report directly to the Substance Use Disorder Program Manager and will work with grant administrators of the program (SAMSHA and Howard University).

    Essential Duties and Responsibilities

    To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

    SUD Assessment Coordinator Responsibilities:

    • Conduct one-on-one interviews and provide educational materials to participants after negotiating GPRA data collection and referral opportunities with providers.
    • Conduct follow-up interviews with participants at 6 months from intake.
    • Coordinate compensation for participants who complete initial and follow-up assessments.
    • Collect, document, and maintain accurate records of interaction with participants at intake and follow-up, and as needed.
    • Assist in developing and revising policies and procedures to guide electronic medical record modifications, program planning, implementation, and evaluation.
    • Monitor and review electronic medical record reports to ensure accuracy of data and follow-up activities.
    • Work with electronic medical record, health information technology, and quality assurance departments to obtain data for periodic reporting.
    • Assist Screening Brief Intervention and Referral to Treatment (SBIRT) champions team with planning and on-going program evaluation (Plan-Do-Study-Act) and outcomes assessment.
    • Serve on the SBIRT champions team, participate in regular meetings, and assume responsibility for assigned tasks.
    • Transmit data collected including GPRA and EHR/EMR data to grantee Data Manager.
    • Meet periodically with grantee Data Manager and Manager of Program Operations to evaluate and provide feedback on data collected.

    Care Coordinator Responsibilities:

    • Complete consent for treatment with participants seeking substance abuse support within 24-48 hours of receiving referral.
    • Provide care coordination both internal and external to Mary's Center in the areas of behavioral health, psychiatry, primary care, dentistry, health specialties, nutrition, public benefits, referrals to education and employment resources, link to housing supports, and additional community resources as needed.
    • Assist Medication Assisted Treatment (MAT) providers and sometimes accompany participants in accessing appropriate level of care including intake at the ARC, detox, and/or inpatient services.
    • Coordinate transportation for participants when necessary.
    • Coordinate the delivery of facilitated telemedicine services for individual participants and/or with partner community agencies.
    • Provide education to participants on how to use Narcan.
    • Work with insurances, MCOs, BHAP, and Buprenorphine Drug Assistance Program to ensure participants receive services and medications in a timely manner.
    • Complete insurance Prior Authorizations when necessary.
    • Identify when participants are in crisis and connect to appropriate team member for further assessment.
    • Schedule appointments, make reminder calls, and outreach participants when they no-show for visits or are lost to care.
    • Provide logistical support to program activities, including recovery celebrations, and community events.
    • Participate in OUD ECHO sessions, weekly meetings, case reviews, and trainings with other team members.
    • Accurately and comprehensively document in the participants chart all encounters, contacts, referrals, and other necessary information within 48 hours.
    • Understand and apply state and federal confidentiality regulations regarding participants' records and correspondence.
    • Perform other duties as assigned.

    Qualifications

    Knowledge, Skills & Abilities -

    • Knowledge of principles, contemporary practices, and trends of health education, interviewing methodology, and individual and group dynamics.
    • Interviewing and reflective listening skills.
    • Demonstrated ability to work collaboratively and communicate effectively with culturally diverse populations, health care professionals, departmental leadership, and community-based and private agencies.
    • Demonstrated experience using Windows Operating Systems including Microsoft Office, e-mail, and Internet.
    • Proven ability to organize and set priorities, work independently, and take initiative to meet deadlines.
    • High School Diploma or GED required. Bachelor of Arts or Sciences preferred.
    • Experience with data management and/or previous work in healthcare, preferably with substance abuse or mental health, setting required. A thorough understanding of privacy and confidentiality regulations is essential.

    Language Requirements - Ability to communicate effectively in English is required. Additional language proficiency or fluency in Spanish preferred.

    Physical Demands - Regularly required to sit; frequently required to reach with hands and arms, walk, stoop, kneel, crouch, talk or hear; must be able to lift objects up to twenty-five (25) pounds.

    Work Environment - Mostly in a typical office and/or tele-work setting with quiet to moderate noise level. May be exposed to extreme cold, heat, and humidity due to outside weather conditions.
