Illinois Youth Investment Program Job Description and Expectations
The primary responsibility of the Case Manger (CM) is to provide community education as needed by the agency.
The CM will also be responsible to participate in other Haven programs and will accept other responsibilities as assigned by the Executive Director.
Minimum Qualifications:
Degree in Social Work, or a comparable degree in mental health services.
Direct Service
The CM will be responsible for providing direct care to clients individually, and in a group format. On average, this position will carry a caseload of approximately 8-10 clients.
- Identify and interview eligible youth for Haven's, Illinois Youth Investment Program (IYIP).
- Assist students and families with obtaining required documentation to enroll in the HYFS IYIP program.
- Complete and submit all reports to the Sate regarding program outcomes.
- Complete thorough assessments of each participant, identifying a chosen career pathway and potential barriers to successful education and career outcomes.
- Develop individualized case plans for each participant that address academic concerns, short
- Submit enrollment, discharge and monthly IYIP paperwork as required.
- Assist in the development of short and longterm plans for programs, monitor progress, assure adherence, and evaluate performance of programs and work sites.
- Establish partnerships within the community and county that are willing to provide job shadowing, internship, and employment opportunities to IYIP participants.
- Work collaboratively with employers and site supervisors to establish, monitor and evaluate participant's work experience and ensure that all policies, protocols, goals, and standards under the program are being met.
- Travel to partnership work sites and conduct weekly checkins and onsite visits once placement has occurred.
Quality Assurance
The CM will provide weekly supervision to clinical staff.
- The CM will assist with file supervision and compliance, reviewing charts and signing off on intake assessments, treatment plans and termination/aftercare plans.
- The CM will facilitate ongoing communication with referral entities to address program concerns and develop solutions to increase satisfaction for all collaborating parties.
- The CM will be responsible for understanding funding sources and program requirements.
- The CM will assist with monthly reports of treatment outcomes, reviewing number of referrals, intakes and discharges.
- The CM will conduct team meetings and relay relevant information between all programs.
Training
The CM will attend all program meetings at local and State level.
The CM will assist acclimating interns to the counseling programs.
Pay:
$50, $53,000.00 per year
