As an Addus/ A Plus Service Coordinator, you will:
Coordinate and schedule the most appropriate assignments of direct care staff to clients. Maintain direct care staff schedules in the scheduling program.
Supervise and direct activities of HomeCare Aides (HCA) in providing direct care and service to their assigned clients. Address and resolve client and employee complaints and concerns
Effectively manage the communication and relationships among the Care Team, including the client, referral sources, HCAs and family and health care providers, to coordinate patient care and services.
In partnership with the Branch Director, provide feedback on performance and coordinate disciplinary actions.Understand and effectively execute all company, state and applicable CBA policies, procedures, rules and regulations, including Covid protocols.
Position Requirements & Competencies:
Must have high school diploma or equivalent or Some College.
Interpersonal, organizational and communication skills.
Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
Must have reliable transportation.
