Job Description:

Plans, organizes, and coordinates all aspects of the Home Care Network's performance improvement program. Reviews and recommendations program improvement opportunities and promote efficient utilization of resources.

Job Responsibility

  • Collaborates with System Quality Management vendors; Hospital Quality Management staff; Health Information Management (HIM); Finance and Information Systems departments.
  • Assists System hospitals with report manipulation, interpretation and data analysis.
  • Supports Quality Management leaders relating to measure definitions, reports, and webbased tools.
  • Provides education and guidance, and prepares educational materials related to Quality Management.
  • Participates in System Quality Management performance improvement task forces.
  • Researches and prepares material and reports needed for quality management activities.
  • Prepares minutes for various Quality Management meetings.
  • Compile data and prepare PowerPoint presentations as necessary.

Job Qualification

  • Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
  • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
  • 13 years of relevant experience, required.
