Intake Coordinator Job description:

Intake Coordinator Responsibilities:

Initiate contact with prospective clients, conveying our services effectively to address their home care needs.
Collaborate seamlessly with the Coordination team and other intake team members, ensuring a cohesive and efficient workflow.
Accurately enter and maintain data on inquiries spreadsheet or HHA exchange platform daily, fostering organizational efficiency and data accuracy.
Assist in the preparation of business intelligence reports, offering insights into key performance indicators related to calls, intakes, referral sources, and more for informed decisionmaking.
Ensure precise entry of patient information to maintain a high standard of accuracy, crucial for providing personalized and effective care.
Demonstrate a respectful and nondiscriminatory approach in all interactions with referral parties, patients, and staff, prioritizing a positive and inclusive experience.
Proactively engage in work with a solutionoriented mindset, contributing to a proactive and forwardthinking work culture.
Uphold compliance with all State and Federal regulatory requirements, as well as M&N Home Care policies related to the referral and intake process, ensuring legal adherence and ethical conduct.

Knowledge, Skills, Abilities, and Other Attributes(KSAO's)

High school diploma required; Bachelor's degree or equivalent professional experience preferred.
Impeccable attention to detail coupled with a natural curiosity.
Ability to craft professional, courteous communications with flawless spelling and grammar.
Proven capability to proactively selfdirect and selforganize.
Experience working with the HHA Exchange Platform is preferred.
Track record of handling sensitive and confidential information with the utmost professionalism.
Strong customer service orientation.
Proficiency in Hindi, Ursu is a MUST.

Pay:
$ $22.00 per hour
