Responsibilities:


  • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.
  • Coordinates the establishment of the initial plan of care with the attending physician, m edical d irector and the i nterdisciplinary t eam.
  • Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of p hysician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates significant findings, problems, changes in condition or environment to the c linical m anager/ p hysician and other members of the hospice care team. Communicates with the p hysician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
  • Performs oncall responsibilities and oncall services to patients/families as assigned. Performs other duties as assigned.

Qualifications:


  • Current RN license, specific to the state(s) you are assigned to work.
  • One year of clinical RN experience.
  • Current CPR certification.
